Emergency Contact Information
If a disaster occurs, you may not have immediate access to phone numbers or utility account information that you may need. Use the Emergency Response Contact Information Sheet to document this information for each of your organization's locations.
The form includes spaces to document your emergency response team leader and contact information as well as utility account and phone numbers. If you have an alarm, also document the associated company's phone number.
If your organization has designated an alternate working location in the event the current one is not available, document it on the form as well. You may also wish to designate an evacuation meeting place for all employees to gather outside if your building must be evacuated in a hurry.
You may wish to distribute copies of this form to multiple individuals or maintain copies at several locations to ensure the information is readily available.
